Overview

Open Sky Community Services is a human service agency that provides counseling services and treatment options to people diagnosed with a mental illness or developmental disability. Under the direction of the Day Habilitation Director , The Day Habilitation Service Manager is responsible for treatment plan development and case management of assigned individuals. The Day Habilitation Service Manager is a leadership position providing supervision to assigned staff and covering in Program Director's absence. Wrentham Community Connections is a Day Hab program currently assisting adutls between the ages of 23-66 with a primary diagnosis of intellectual disabilities. Most participants require supportive structures and staff assistance to maximize their involvement in activities in the building as well as in the local community. Services and supports provided to participants include, but are not limited to, occupational therapy, speech therapy, physical therapy, nursing oversight, functional skill development, communication, socialization, community membership and self-advocacy.

Responsibilities

  1. Oversee services for supported individuals.
  2. Ensure compliance with Mass Health and DDS regulations
  3. Develop, coordinate and oversee the implementation of Day Hab Service Plans (DHSP) and supported routines.
  4. Hire, supervise, mentor and evaluate staff.
  5. Train staff in the goal writing and implementation of strategies.
  6. Provide leadership, supervision and support to staff team.
  7. Ensure staff meet all training requirements.
  8. Develop, implement and monitor systems for quality improvement.
  9. Serve as a member on committees.
  10. Maximize the development of community-based programming and volunteer activities.
  11. Assist in daily programming by providing group coverage, crisis intervention and other related tasks as needed.
  12. Ensure physical maintenance and cleanliness of the program.
  13. Provide transportation as needed for supported individuals.
  14. Review and complete all required documentation in a timely manner.
  15. Communicate pertinent information to all stakeholders.
  16. Foster cooperative relationships and agency values among all staff, people we serve, their sponsors and families.
  17. Implement and support all agency and program policies and procedures
  18. Attend and participate in trainings and maintain required certifications


Qualifications

High School Diploma or GED and at least 4 years’ experience working with individuals with Developmental Disabilities, 2 of which in a supervisory role; experience with case management and care review in a healthcare setting and valid driver’s license required.
